Hudson Schools Online Bullying/Harassment Form

The Hudson Community School District cares about the safety and well-being of all students. We want to make sure it is the best possible place for students to learn. The Board of Education and staff realize bullying/harassment is detrimental to the health and safety of students and disruptive to the educational process. Please refer to Board Policy 104, located on page 23 of the student handbook for more information. This form will be used to assist in the investigation of incidents that may not be otherwise reported directly to an adult. Names are required only schools may complete a thorough investigation. Please understand that the results of all investigations are confidential. However, you can expect to receive contact following the reported incident to both acknowledge receipt of the report and for the investigator to collect additional information when necessary.   • In the space provided, please tell us in as much detail as possible the circumstances surrounding this issue.
  • If this has previously been reported below, please describe actions that were taken to resolve this problem.
  • State law requires that all reported cases be investigated.
