Required Immunizations, Medical Forms

1. Preschool: Current immunization record, copy of physical (from within current calendar year).
2. Kindergarten: Current immunization record, copy of physical (including lead screening and vision testing results), dental screening.
3. 7th grade: Tdap vaccine for students born after 9/15/2000. Meningococcal vaccine for students born after 9/15/2004. (Both immunizations are required and there is no provisional.)
4. 9th grade: Dental screening form to be completed by dentist. 
5. 12th grade: Students born after 9/15/1999 shall have two doses of meningococcal vaccine; or one dose if first dose received when student 16 years of age or older. (Both immunizations are required and there is no provisional.)

There are certain immunizations and health records that are required as part of the school registration process. This brochure outlines the immunizations that are required for students as well as the types of exemptions that are available. Students who are not immunized or have exemptions on file with the district are not permitted to attend school.
